cannabis cardTo obtain your cannabis card in Sacramento, you must apply for it with the county. You can pick up an application at the Sacramento County Office of Vital Records. Their address is 7001 East Parkway Suite 650. The hours are Monday through Friday from 8 am to 5 pm. The applications are available in English and Spanish. You can also go to the California Department of Public Health’s online site and print out an application, or call (916) 875-2066 to request an application by mail.

Once you have completed your medical cannabis card application, you must make an appointment to formally apply by calling (916) 875-2066. There is a $100 non-refundable application fee, and appointments are only scheduled on Wednesdays and Fridays between 9 am and 4 pm.

Along with the completed application and application fee, you will also need to bring a California driver’s license or government-issued photo ID. You must also prove residency by bringing your motor vehicle registration from the state of California, a utility bill, or rental receipt.
